Pilot Program Improves Health Outcomes, Suggests Potential to Reduce Health Care Costs
Beacon, Care Coordination Partners Help Chronic Disease Patients
Double-digit improvements in health outcomes are among the results of a recent care coordination pilot program led by Hawai‘i Island Care Coordination Services (HICCS), a joint venture between Ho‘okele Health Innovations and West Hawaii Home Health Services, with the support of the Hawai‘i Island Beacon Community (HIBC). Data showed a significant impact, including:
- 36 percent improvement in HbA1c blood sugar
- 37 percent improvement in triglycerides
- 29 percent reduction in inpatient hospital admissions
- 25 percent reduction in emergency room visits

HIBC Achieves Standout Health Care Improvements on the Big Island
84 Percent of Primary Care Physicians Now Using Certified Electronic Health Records.
Three years after entering into a $16.1 million cooperative agreement with the Office of the National Coordinator for Health Information Technology (ONC), the Hawai‘i Island Beacon Community (HIBC) reports significant achievements in the use of innovative technology to improve the quality of patient care in Hawai‘i County. As one of the most rural and geographically isolated of the 17 Beacon Communities and in the face of unique challenges – geographic distances, diverse populations, and limited health care resources – HIBC is making improvements toward better health, better care and lower costs.View full release
