IT and Care Transitions Videos Featuring Beacon Communities Now on ONC YouTube Channel

Many of the Beacon Communities participated in interviews for IT and care transitions videos, during a Beacon meeting last fall. ONC just posted these five new videos on the ONC YouTube Channel! Watch these videos to discover and share with stakeholders the value proposition around several of the Learning Guide themes, including improving care transitions through health IT. All of the videos are a succinct two to four minutes long, and serve as an excellent resource to promote and raise awareness for how health IT can be used to improve health and health care.

ONC guide aims to help providers use EHR alerts to reduce readmissions

The Office of the National Coordinator for Health IT’s Beacon team recently released its first guide to help interested stakeholders learn from the experiences of the Beacon communities in implementing electronic health records. The first guide, “Improve Hospital Transitions and Chronic Disease Care Management Using Admission, Discharge and Transfer-Based Alerts” is “designed for communities that have a stated goal to reduce avoidable ED visits, avoidable hospitalizations, and preventable readmissions and have identified the implementation of ADT-based alerts as a potential strategy to achieve the goals,” according to the authors. Beacon Communities Learning Guides

The HHS Office of the National Coordinator for Health IT (ONC) provided $250 million over three years to 17 selected Beacon Communities throughout the United States. The Beacon Nation project is converting the experiences and lessons learned from the Beacon Communities into actionable information about specific technology-enabled solutions that can be adapted for use by interested communities. This information is included in a series of Learning Guides, a common set of materials describing a promising IT-enabled intervention that can be deployed in a community to accelerate change. Mostashari: Beacons Highlight Needed National Action

Mostashari: Beacons ‘Taught Us’

National Coordinator for Health IT Announces Release of the Beacon Nation’s First Learning Guide

On May 22, the National Coordinator for Health Information Technology, Dr. Farzad Mostashari, announced the release of the Beacon Nation project’s first Learning Guide: “Improve Hospital Transitions and Chronic Disease Care Management Using Admission, Discharge, or Transfer (ADT)-based Alerts.” 

This Learning Guide draws on insight, experiences, and lessons learned from the Beacon Communities and contains concrete technology-enabled solutions that can be adapted for use by communities across the nation. It was released at “The Beacon Community Experience: Illuminating the Path Forward,” a session hosted during the three year anniversary of the Beacon Community Program.

A series of Learning Guides covering various relevant health IT topics will be available in the Learning Guide section of the Beacon Nation web site as they are completed.
